Ah, thanks Marcy. My bad, I forgot we had that on LPSF.org. I do
think the form we're using there is too long -- it would be better
condensed into no more than half a page, rather than a full page --
and is also somewhat confusing and not very visually attractive.
However I'm not sure I want to take on trying to fix it right now.
